Steps to Apply
- Present Evidence of Death Certificate of Guardian/Parent
Students whose parents or guardians have passed away are required to provide an official death certificate as proof of the incident. - Provide the Medical Cause of Death
Alongside the death certificate, a document indicating the medical cause of death should be submitted to the SRC office. - In Case of Disability, Provide Evidence of Disability of Guardian/Parent
If the application is due to the disability of a parent or guardian rather than death, students must present valid documentation confirming the disability. - Attach a Letter of Request
Write and submit a letter requesting the insurance claim. Address the letter to the SRC President and ensure it includes all relevant details regarding your claim. - Print a Screenshot of Your School Fees
As part of the documentation, applicants are required to provide a printed screenshot of their school fees to confirm their student status.
